Already on a released map.
See that bright patch on the ground? It's a distortion. At the time it wasn't a BSP hole or anything so I wasn't worried. You can walk over it fine but it tends to do this to the visuals:
I've seen this kinda thing before but not for a very long time.
What is it? More importantly, how would I go about fixing it??
What is that and how do I fix it?

It's a BSP cut, a real nasty one. that part must be rebuilded with proper brushes.
Most likely also a wrong single vertex edit would cause this.. Certain surfaces aren't "straight".I encountered this many times too during my mapping projects. Especially the first crystalmine (building the ''mines" ) had these all over. Remove and rebuild with proper brushes will fix it.
And the definition of a bsp cut means that it's surface is not straight and lets you see trough outside the map, but out of range perspective.

Sadly a rebuild is currently out of my range of skills. Still, I might be able to align it better. Worth a shot.
