Many maps in UT have exhibited a lot of Gothic style themes in the architecture, and i've seen this head mounted on buildings before.

Yeah, as far as what the image is OF - Scify or Sir Mandrake offer a good one.
Vulture seems good, but with a bit of Gothic flair.
Folks - in case you don't know, this is an image used by the UT map editor to show certain actors used in maps, such as lift centers and triggers, etc.
These are invisible when playing a map.
